No appointment needed — just walk in during office hours.
Operator Requirements (LTFRB)
Mandatory Requirements:
- LTO original & photocopy of Certificate of Registration (CR)
- LTO original & photocopy of latest Official Receipt (OR)
- Picture of the unit (front, back & 2 sides) with current newspaper
- Proof of Financial Capability – Php20,000 per unit (Bank Certificate or photocopy of Passbook)
- Year Model allowed NOT more than THREE (3) years old from date of manufacture. Maximum age limit of SEVEN (7) years
- Photocopy of Government Issued ID with 3 specimen signatures
- OPPA RIDE Registration Form (c/o TNC)
- Clear copy of 2 x 2 picture with white background
- Signed and Notarized SPA (c/o TNC)
- Notarized Certification of Authenticity (c/o TNC)
- Application for Certificate of Public Convenience with Prayer for Issuance of Provisional Authority
- Passenger Accident Managers, Inc. (PAMI)

- Valid Professional Driver's License (original & photocopy)
- NBI Clearance (original & photocopy)
- Police Clearance (original & photocopy)
- 2 passport-size photos (white background)
